Introduction to the Brow Lift (Forehead Lift) Procedure

By Daniel Ebroon There are several established methods for performing an eyebrow (also called a forehead) lift. The oldest method is called a coronal brow lift. This form of a brow lift is done by making a very large incision behind the hairline from one side of the head to the other. The forehead and brow are lifted accordingly and the incision is sutured closed. The most important advantage to this form of a brow lift is the fact that the brow can be raised a significant amount by simply removing more and more skin. However, this form of a brow lift also has several very large disadvantages. The surgery is much more expensive than other forms of a brow lift. Also, there is a very large scar hidden behind the hairline in those with abundant hair. Those who do not have thick hair do not hide the incision well. Also, there can be permanent numbness in the scalp region after this form of surgery, as some of the sensory nerves are cut by the surgical technique. Other forms of a brow lift or forehead lift are performed without making such a large incision. One such form of a brow lift is an endoscopic brow lift. This is done by making several small incisions behind the hairline and using a small camera inserted through one of the incisions, the forehead is lifted to a higher point. The advantage of this form of a brow lift is that there is no single large incision. However, the incisions still can be visible if the patient does not have a full head of hair. The technique employs various methods of keeping the forehead in a higher position once it has been lifted through the small incisions. One of these methods is a suturing method that holds the brow and forehead up higher. The other employs a device called an Endotine, which is a fixation device made of a dissolvable material that holds the brow and forehead in its higher position for three to six months before dissolving. The forehead and brow form natural attachments over this period of time that keep the brow in the new higher position. Another method of raising the brows is to do what is called a transblepharoplasty brow lift. This is a method of raising the eyebrows through the upper eyelid incision that is made for a blepharoplasty. Since brow lifts and blepharoplasties are usually performed together, sometimes this means that the brow can be elevated without an additional incision. With this method of a brow lift, the brow and forehead are addressed from the eyelid incisions. They are lifted and raised to a higher position. Just as in the endoscopic forehead lift, the brow and forehead need to be fixated in the new higher position and this can be accomplished either with sutures or with the device called the Endotine. The advantage of this form of a brow lift is that the incisions are minimal. This allows for much faster healing and essentially no visible scars. The disadvantage of this technique is that the eyebrow and forehead may not be able to be elevated as high as with the other forms of a brow lift. In many patients the tail of the brow will descend to a more significant degree than other portions of the eyebrow. There are many patients in which Dr. Ebroon would advise raising the tail of the brow much more than the central portion of the brow or the portion closest to the nose. Raising the tail is helpful because it can give the effects of a brow lift without giving the surprised look that no patient is interested in. The tail of the brow can be lifted with any of the methods outlined above. Usually, after consulting with the patient, your physician might recommend a transblepharoplasty assisted brow lift, perhaps with a single incision in the temple region, to achieve the desired brow position. Brow lifts are an important part of eyelid and upper face rejuvenation. A subtle brow lift can add a tremendous amount to the effects of an upper eyelid blepharoplasty. It can enhance the rejuvenating effects of an upper eyelid blepharoplasty and is therefore a very popular procedure. Some patients prefer to raise the brows non-surgically. This can be done to a small degree with the use of Botox. Botox is a medicine that temporarily relaxes muscles, usually for a period of three months. It is a very popular method to reduce wrinkles between the eyebrows, in the forehead, and in the crows feet areas. Botox can also be used in the tail of the brow to raise the tail of the brow minimally. It has this effect because there are muscles near the eyebrows that both raise the tail of the eyebrow (the frontalis muscle) and muscles that depress the tail of the brow or bring the tail of the brow down (the orbicularis muscle). If Botox is skillfully placed into the orbicularis muscle in the area in which it brings the eyebrow down, this muscle will temporarily be weakened and the muscle that raises the tail of the brow will work unopposed and perform a nonsurgical temporary minimal brow lift. Depending on an individuals personal anatomy and their personal feelings about surgery, the appropriate method can be employed to enhance the brow position and thereby enhance appearance. The gift should be something that says I am thinking about you or I enjoy the time we spend together. It is important to stay away from buying rings as a gift since this may be seen as a serious gift. Generally rings should not be purchased unless you intend to propose marriage to the woman. Dating rules are not set in stone they are merely suggestions. Different rules work for different people so you may have other ideas on dating and gift giving. Men can give women a gift at any point in the dating relationship but depending on the gift or the timing, the gift may be seen negatively. It is better to spend the money on a good time that both parties can enjoy together as opposed to a gift. After you have been dating awhile it is perfectly all right to buy your dates a small gift but save the big more expensive gifts until you are ready for making a lifetime commitment to one another. Tips For Brainstorming

By Jay Hamilton-Roth “A group problem-solving technique that involves the spontaneous contribution of ideas from all members of the group” — Merriam-Webster Dictionary Brainstorming is one of many creative problem solving techniques. While you can use any size group, we believe it works best when done in a facilitated group of 5-10 people. The more diverse the group the more diverse the input. The group members don’t need to be experts in your field. In fact, they don’t need to know anything about the problem topic at all! The following steps I use in my Many Good Ideas Brainstorming Seminars: Step 1: Define Your Problem You need to get really clear on what your problem truly is (and is not). Keep asking “why?” until you feel that you’ve identified the root of your problem. For example, if your problem is “I need more money?” Ask “Why? To do what? And then what?”. Once you have the problem, state the brainstorming topic as: “I would like to _________ but it’s difficult because ________” The more time you spend in defining your problem the better the results will be. Step 2: Uncover Solutions Now is when the group starts contributing ideas. The group should some pre-agreed rules. Here are some: I will listen to others’ ideas. I will not be attached to my idea. I will attempt to find the good in someone else’s ideas. I will give myself permission to be wrong, insane, and stupid. I will respect that the person with a problem is best judge as to quality of the idea. I will not tell someone else what to do, I will only offer suggestions. Make sure you have someone responsible for recording the meeting (that person should definitely not be the person who’s presenting their problem). Brainstorm for a set time. Initially, a lot of obvious ideas will be suggested. After this initial flurry, things will quiet down. Be patient. Risk saying something that makes no sense to you. The better ideas usually arrive after people have relaxed and listened to others. Step 3: Evaluate Solutions Here is where the solutions that were suggested can be evaluated. What ideas are the most exciting? If any of the ideas seem incomplete, go back to step 1 and re-brainstorm. Remember that brainstorming can be an intensely intimate encounter. DUI Arrest Urgent First Steps

By Tim H When you’re first arrested for a DUI or drunk driving charge you are given a literal TON of conflicting information from the courts, the DMV, DUI lawyers, and of course the good old internet. So this article will hopefully serve to clarify a few things for you so you can make the best decisions for yourself and your future. Of the most urgent concern for you to now think about is preserving your driving privileges with the DMV. Go through all the paperwork you have that the police officer gave you the night of your DUI arrest. What this will do is extend your driving privilege until the hearing actually takes place. Depending on how backed up the DMV is with these driver license hearings it could be months… and you will be allowed to drive for these months. So the point is find that piece of paper (the paper license) and locate the telephone number on the back that instructs you as to exactly how to schedule your DMV administrative hearing. After you have successfully scheduled your hearing you’ll then want to consider defending yourself from being convicted. Why Good Partnerships Go Bad

By Dennis McCue One of the most common structures for law firms is a business partnership. There are many reasons why, but if you consider that it is probably the most difficult arrangement for doing business, you would think that attorneys some of whom are the experts in the creation and dissolution of business entities would avoid them. Yet this is not the case. In fact, some lawyers join and leave partnerships more often than some people buy new cars. What is a partnership? A partnership is a voluntary relationship between individuals or entities, characterized by cooperation and responsibility and intended to focus on a common goal. It is a contract between competent persons for pooling their money, goods, labor and/or skill with the agreement that profits will be divided between them, and that they will use the partnership entity to conduct business for their mutual benefit. In the qualitative sense, partnership is characterized by accord, affinity, collaboration, fraternization, support and friendship. Many law firms are known for being viable partnerships that work to represent the interests and skills of each shareholder in the best possible manner. Some firms even evidence that spirit of cooperation that makes a partnership incredible and fun bringing profit and goodwill to all the participants, including staff and clients. This, obviously, is the ideal firm. However, firms do not usually start out that way, and many never even approach it. Why do so many firms fall short of the ideal? The short answer, of course, is because there are humans involved. Nevertheless, it might be helpful to consider that the people who become attorneys have a determination that propels them into law school and to do what is necessary to pass the bar exam. This process of education and examination hones their innate drive into a focused, goal-oriented person. This places members of the legal profession among the most educated and trained in our society. When a new lawyer passes the bar, what does the future hold? Often it is as an associate in a firm. This is where the real training begins. Associates are expected to be able to think on their feet, to do whatever partners do not want to do, to adapt to rigid rules of the court, and to substitute for other attorneys at the last minute with little opportunity to prepare and little knowledge of the case they are representing. All the while, order and decorum must be maintained. This training process helps them to become great client advocates and when you add the billable hour expectations of many firms it also builds their endurance (tolerance for pain). Over time, the process yields its likely product an attorney with a warrior persona, prepared to subjugate their own needs in the service of their clients. This is the high and noble calling of the profession. Concurrent with all this training, the internal motivation of the person continues to develop. While the qualifying process is taking place, their initial motivation, drive or vision is becoming strengthened as well. Often kept at bay with the promise of future rewards partnership, private practice, social status and/or better-than-average compensation, this drive must come into self-expression. Goal-driven, status seeking and reward focused, the next logical step for many is either partner or private practice. The strength of character required is similar, but it is at this point that the challenges diverge. Our purpose here is to address the challenges facing the partners in firms. Making partner is a goal for most associates and, for them, it is as important often more so as marriage. Of course, it also brings all of the obligations, but very different rewards. Like the marriage ceremony, partnership brings on a new era filled with new duties that have to do with the successful running of the firm itself and with meeting the expectations of the other partners. It is here that the groundwork is laid for the future of the firm. Large firms have developed cultures that incubate associates to fit in. Those who do become partner. The others leave the firm. Smaller firms, however, are made up of lawyers who either did not fit into the mold of the larger firms or wanted more control over their destiny. Law school friends, who trained in different firms and disciplines, may decide to begin a practice together. A few associates in a practice area of a large firm may opt to begin a boutique firm. Regardless of the course of their arrival, the attorneys who become the partners of small and medium firms, immediately face two new challenges in addition to the need to generate and service their own book of business cooperation with the other partners to mutual objectives and fulfilling their share of the obligations of managing the practice. However, nothing in their training has prepared them for meeting the shared, yet sometimes clashing interests and expectations of their partners. Law school does little to prepare attorneys for the business necessities of running a practice. Most law firms do not train the associates in it either. Up to the moment of becoming a partner, the attorneys career has been largely a solitary performance and the rewards have been commensurate with it. It is at this time that the repressed, but strong internal motivators combined with the warrior persona tend to rise to the surface. Without the training for business, this personality does not bend easily toward cooperation even when appropriate and beneficial to his or her own interests. Partnerships are formed for a variety of reasons, usually based more on hope than research. As partners, financial rewards are based upon the shared performance of all partners. However, often the partners do not share the same objectives or circumstances. Sometimes the approach to fees is similar. Other times it is drastically different. Firms are founded where the partnership may consist of a litigator, a family practice attorney, a personal injury lawyer and an insurance specialist. If one of the partners is particularly successful in bringing in cash, resentment and discord quickly become antagonism and outright warfare. These are the antithesis of true partnership. Recognition, Resolution and Growth All firms must face these challenges at one time or another. The largest firms have confronted them and designed solutions to address them, enabling growth and prosperity for partners with a divergence of talent and skills. Smaller firms have a harder time due to the perception of a lack of time and funds to overcome the problems. The keys to passing through this phase: recognition, telling the truth about it, a commitment to the growth of the firm above self-interests, communication with your partners so that everyone wins, finding the right combination of new business generation, client service, firm management and partner relations. Unfortunately, there is no guaranteed winning formula. Each partnership must face the challenges and design its own solutions. Generally, when mutual respect and acknowledgment of the value of all aspects of work done for the firm are the rule, solutions can be designed to enable firms to grow beyond these difficulties.
